Database on the promotion of professionals to the title

The database shall contain the following information: personal name (forename, surname), tax number, date of birth (day, month, year), place of birth (place, country), residence (permanent, temporary; municipality, settlement, street, house number), educational institution of employment (name, registered office, responsible person), work place(s), education (level and course of education at individual levels, professional title, type, issuer and number of public documents proving it), professional examination (field), data on work experience outside education and training, data on length of service (full period of service, length of service in education, training), performance data, data on the existing title (type, date of award), data on professional training (title of the programme, duration and duration and number of points), details of additional professional work (description, time of performance, evaluation), professional worker’s consent, information on the title awarded (type, award date). The data relate to professionals in the field of education and training and are collected for the purpose of exercising the right to promotion to the title.
